Use this form if directed to do so by Religious Education staff.

• Enter the amount to be paid in the “Fee” box below.
• Include a description of the payment in the memo field.
  • For the Sunday Morning RE and Confirmation Prep programs, indicate how many students/candidates for each program. For example, “4 Sunday Morning RE students and 1 Confirmation Prep candidate.”
  • If you have arranged to pay a fee on a schedule, please note what the schedule is. “$300/mo until paid” or “half now, half next month.” Return to this page to make future payments.
  • If you are paying a reduced fee please indicate so. For example, “catechist fee” or “reduced fee per conversation with Anne Marie”. Do not provide any personal details on this form.
