Faith Formation Enrollment Form
Use this form to enroll children in the following Faith Formation programs:
A few things to know before you get started:
- All registration and payment for faith formation programs and sacramental preparation for children and youth will use this one form. This includes youth ministry.
- If you are using a personalized url that was emailed to you, please look over the prefilled information (address, emails, phone #'s, etc.) to make sure it is correct and up to date.
- If you being filling out the form and are unable to immediately complete it, you are able to "save" it and complete it at a later time. When saving the form, it will not be submitted to us. In order to submit it, you must use the Submit button on the last page. When "saving" the form, you will be prompted to create a JotForm account. You do not need to do that if you don't want to. Below that option, in small type there is an option to "skip account creation". Click that and you'll be given the option to get a link to your saved form.
For Religous Education programs,
- Enroll by July 15, 2023 to receive the early bird rate of $200/child.
- Complete enrollment by August 15, 2023 to guarantee your child's placement in the Sunday Morning RE program. Fee: $300/child.
- After August 15, 2023 children will be placed as space and catechists are available. Fee: $300/child.
- New families: $200/child (regardless of enrollment date).
- If you are currently unable to pay the full amount, please know that scholarships are
  available. Please call Anne Marie Kaufmann at 202-903-2832 for more information about financial support to enroll in our faith formation programs. (Start filling out the form and save it. Submit after talking to Anne Marie.)
For the First Reconciliation/First Eucharist Preparation program (for 2nd grade and older):
- Please review the requirements on the Holy Trinity sacramental readiness page
  (https://trinity.org/faith-formation/sacramental-preparation/), before completing this form. If your child does not meet the requirements or you are unsure of his/her readiness, please contact the Office of Faith Formation.
- Children preparing to celebrate First Reconciliation & First Eucharist are required to be enrolled in a faith formation program either through Holy Trinity Parish, Holy Trinity School, or another Catholic school for at least one year prior to the sacramental year, as well as during the sacramental year itself.
For the Confirmation Preparation program (for 8th grade and older):
- Please review the guidelines for Confirmation on our Confirmation Preparation page. Only register your child for Confirmation preparation, if s/he is in 8th grade or older.
- Young people preparing to celebrate Confirmation are required to be enrolled in a faith formation program either through Holy Trinity Parish, Holy Trinity School, or another Catholic school for at least one year prior to the sacramental year, as well as during the sacramental year itself.

Volunteering
The continued success of the Faith Formation program at Holy Trinity rests on the involvement and participation of parents and parishioners. We are all asked to give freely and generously of our time, talents and treasure.

At least one parent/guardian from each family is asked to volunteer in some capacity amounting to a minimum of 4-5 hours of total service, as the burdens of many activities cannot be successfully supported by the shoulders of a few. We will do our best to accommodate your preferences and to incorporate your particular talent, experience, and knowledge in the most appropriate way. Please consider making a donation to youth ministry at Holy Trinity. While we do collect fees for some events throughout the year that may have an associated cost (such as ice skating) from those teens who participate, there are additional expenses such as providing food at weekly meetings for which we do not charge.
