Use this form if directed to do so by Faith Formation staff.

• Fee: Enter the amount to be paid for Sunday Morning or At Home Faith Formation and Confirmation Prep in their corresponding “Fee” boxes below.
• Memo: Include a description of the payment in the memo field.
  • For the Faith Formation and Confirmation Prep programs, indicate how many students/candidates and their names.
  • If you have arranged to pay a fee on a schedule, please note what the schedule is. “$300/mo until paid” or “half now, half next month.” Return to this page to make future payments.
  • If you are paying a reduced fee please indicate so. For example, “catechist fee” or “reduced fee per conversation with Anne Marie”. Do not provide any personal details on this form.
