Activity Permission & Emergency Medical Release Form
If you intend to participate in Youth Ministry activities on site at Holy Trinity or virtually, please complete both sides of this form and return to the Coordinator of Youth Ministry.
School Service Hour Requirement Assistance
Students looking to complete a service hour requirement for school can do so through Holy Trinity's many opportunities. Rudy Dehaney, or the Coordinator of Youth Ministry, will act as the "supervisor" and sign off on all online or paper service hour forms, regardless of the type of service. You can email the forms to rdehaney@trinity.org with at least 3 days notice so he can fulfill requests in a timely manner.

- Our Migrant Familia has various events throughout the year to get involved in. Email migrantteam@trinity.org for information.

This is a direct service opportunity good for families and children ages 10+. Parents should accompany children under 16. Please do not show up if you have not signed up on the Sign Up Genius. Make sure each person you're bringing is represented on the Sign Up Genius.

Fr. McKenna Center Service Hours
Sign up for shifts at Fr. McKenna Center in their Clothing Closet, Men's Day Program, and/or Food Pantry.
We Are Family DC Service Hours
Sign up for shifts with We Are Family DC to visit low-income seniors and deliver groceries (that part requires a car/adult driver - great for a family to do together!)
We Are Family has Jesuit roots as it started as a ministry out of St. Aloysius Church (now the chapel for Gonzaga College HS).
